**NOTE: You must complete a Proof of Claim Form and save it as a pdf document in order to file it in CM/ECF. See PDF Document Basics for more information regarding
pdf documents.
- Select Bankruptcy > File Claims
- Enter Case Number (e.g. xx-xxxxx). Leave the Name of creditor text box blank. Click Next.
- Select a Creditor for Claim screen:
- Click on the down arrow to search for your creditor. NOTE: If you find your creditor's name and address in the drop down box, the name and address must match exactly with the name and address listed on your Proof of Claim form:
- If you find the exact match of your creditor, select it by clicking on the creditor's name and then click Next. (Skip to Step #9)
- If you don't find your creditor's name or you do not find an exact match, select Add Creditor. (Proceed to Step #4)
- Click on the down arrow to search for your creditor. NOTE: If you find your creditor's name and address in the drop down box, the name and address must match exactly with the name and address listed on your Proof of Claim form:
- Enter Case Number (e.g. xx-xxxxx). Click Next.
- Adding a Creditor screen:
- Type the exact information that is listed on the pdf Proof of Claim Form; Creditor's name, address, state, city, and zip code in the Name and Address text box.
- Leave Creditor type to Creditor.
- Leave Creditor committee at No.
- Click Next.
- Review the Total Creditors Entered, which should read one (1).
- Click Submit.
- Click File a Proof of Claim on the Creditors Receipt screen.
- Enter Case Number (e.g. xx-xxxxx). Leave the Name of creditor text box blank. Click Next.
- Select a Creditor for Claim screen:
- Click on the down arrow and search for the creditor you just added.
- Select your creditor by clicking on the creditor's name and then click Next.
- Proof of Claim Information screen:
- Enter the total amount of the claim in Amount Claimed. Also, if applicable, enter the amounts that are secured or priority.
- If required or needed, enter a description and/or remarks in the appropriate text boxes.
- Click Next.
- Select Choose File to browse and select the pdf file. Click Next.
- If you have attachments to your Proof of Claim, check the box for attachments and proceed to file the attachments.
- Click Next once you have attached all the correct pdf documents.
- REMINDER: Ensure you have redacted all documents with personal identifiers as the documents placed on the docket will be available to the public for viewing.
CAUTION!! This is the last screen. Clicking Next will submit your document to CM/ECF. If the final docket text is incorrect, click the Back button to find the screen to modify or abort the transaction by clicking on Bankruptcy on the CM/ECF Menu bar. - Notice of Electronic Filing/Receipt screen displays
The Notice of Electronic Filing certifies that this pleading is now an official court document.
