Amended Proof of Claim

NOTES:  

  • To file an Amended Proof of Claim form in CMECF, you must complete/save the form as a pdf document, and follow the steps below.  See PDF Document Basics for more information regarding pdf documents.
  • Do not attempt to file the claim to overwrite the incorrect name or address that was used when filing the original claim. Instead, please contact the court for assistance.. 
  • Before filing the claim, ensure to redact personal identifying information described in Fed. R. Bankr.. P. 9037(a). 
  1. Select Bankruptcy > File Claims
  2. Enter Case Number (e.g. xx-xxxxx).  Leave the Name of creditor text box blank.  Click Next.
  3. On the Select a Creditor for Claim screen:
    • Click on the down arrow to search for your creditor.  
    • Select the creditor and click Next.
  4. On the Proof of Claim Information screen:
    • Under the "Amends Claim #" click the Find button (a screen will populate), select the claim that will be amended.  
    • Enter information in all the applicable text boxes, click Next.
  5. Click Browse and attach the Amended Proof of Claim pdf file, click Next
    • If there are documents to attach to the claim, then next to Attachment to Document, select Yes.  Click Next,
  6. The Notice of Electronic Claims Filing screen indicates the claim has been filed.
